We are a family of 4 with 2 teens aged 13 and 16. Noted that many hotels only cater to maximum 3 adults, if I were to book them, will it be big enough for us? will the hotel refuse our stay or made us pay more? We would rather not book separate rooms for costs and safety reasons..

I still recommend you to choose 2 double rooms, cos much wider range of accomodation gets available if it's ok for you to do so.

2 double rooms can be as inexpensive as 1 big room.

As for safety, just put all the valiables in parents' room or hotel's deposit.

Japan is as safe a place as you could ever wish for. Get adjacent rooms/put an adult and a teenager in to share twin rooms. My biggest fear with two that age would be what they might get up to.

Instead of staying in a traditional western hotel, why not try a ryokan? You are in japan after all. If cost is an issue, there are plenty that do not serve meals. For example, in Tokyo, please see:

These are not so much as specific recommendations but a way to let you know the ryokan experience was one of the best parts of our trip for the 4 of us.
