Is it possible that file COE might not be approve? my currently status: waiting for my coe to release.

Yes, of course it is possible. Otherwise, there would be no reason to have the process if every single applicant would get it anyway. It is a process to determine whether an applicant is eligible or not based on a set of conditions.
But note that it can take up to three months (sometimes more) to be processed.
