Pension refund form

Ok, I know there's instructions just about everywhere about the pension refund form and how to fill it out, including the official one.
But I've looked everywhere and can't find one specific thing. I'm American, btw, since I assume bank numbers vary by country:
In the "Bank Account Number" section, do I just need to put the number I would use on a withdrawal slip to my bank, or should it include the routing/transit number as well?

ugh - i am debating the same thing right now. right now i just have my acct. number and next to it i wrote ''bank routing number'' and included that number. i hope it works!
