Yes, a uniform 5% sales tax is charged to your purchases. It is usually (by law) included in the sticker price of an item, except in some situations like the occasional restaurant menu.

as a tourist, am i able to claim the tax back?

Yes and no. Sales tax may be waived at certain authorized stores which advertise themselves as "duty free". The discount is made at the time of purchase, and only if your bill exceeds 10000 yen at one store.
