Change in "Status" of Residence, you mean?

I believe not. Chinese characters are to be used only if applicable.

You can fill out the form in either English or Japanese.

You do not need to fill out the Chinese Character portion of your name.

You only use Kanji (Chinese characters) whenever applicable. If it is not applicable to you, then don't use it.

English on those forms are fine. For your address Romaji translation is generally acceptable.

When I wrote "I believe not," I meant to say "I believe that the application does not need to be in Japanese" :)
