I can hear my neighbout's voice every midnight and it disturbed my sleep. Sometimes I woke up middle of the night due to his loud voice. I'm not sure what is he doing, probably speaking on the phone.
I intend to leave a note on his letter box.
How should I write in Japanese in a polite way?

Are you living in a rented apartment? If so, you should go talk to the landlord/administrator.
