My cousin wants to come to Japan with me. The problem is that he is from one of the Kuril islands. Will he have any problems in Japan?

What do you mean by problems?
Did he get his visa? If yes, I do not see any other problem. Or will he wear a T-Shirts that says "I am from Kuril island"?

Mr. Petrov may fear that his friend will be discriminated against because of the political issues surrounding the two southern Kuril islands which Japan still claim. I think it is extremely unlikely. Japanese prefer to avoid conflicts when possible, and being rude towards a guest is unthinkable.
Legally, I'm not sure. Even if Japan keeps its claim, they still recognize that the islands are administrated by Russia. I don't think there'll be any problems there.
