How to address a letter

I am sending a letter to a friend in Japan, how would I address the envelope?
I think it would be:
Firstname Lastname-Sama
Address
Prefecture
Postal Code
Is this correct? I only know the Romaji and do not know the Kanji form of the address.

Yes, that's fine, but don't forget to add Japan if sending it from overseas:
Firstname Lastname-Sama
Address
Prefecture
Postal Code
JAPAN
I only know the Romaji and do not know the Kanji form of the address.
No problem. Romaji is acceptable for addressing mail.
