Day 1: Narita to New Chitose Airport (probably be arriving at 1:30pm). Stay a night in Sapporo.
Day 2: Day trip to Otaru (Is it worth getting the welcome pass?)
Day 3: Sapporo to Noboribetsu. Overnight in Noboribetsu. Visiting Hell Valley (any other places to visit other than Hell Valley? Possible to visit Lake Toya? Any recommended onsen or ryokan?)
Day 4: Noboribetsu to New Chitose Airport to Tokyo
Day 5: Tokyo (Meiji Shrine/Harajaku area)
Day 6: Disneyland or Disneysea
Day 7: Night flight back to KUL (any recommended places I can do for half day?)
Would appreciate if anyone could give me some advise.

Can you post exact dates? The Otaru Welcome Pass is ok, but it depends a little where you stay in Sapporo and what you do when you return to Sapporo. On Day 7 I'd look at Tsukiji Fish Market, Asakusa/Sensoji/Skytree or Ueno/Ameyoko. They are all explained here:- http://www.japan-guide.com/e/e2164.htmlEnjoy your trip!
