Suggestion for Japan travel itinerary

Dear forumners,

I am planning a trip to Japan with my family consisting of us (husband & wife), 11 year old daughter and elderly parents (both late 70s). Can anyone suggest a travel itinerary for travel from 12 Dec to 21 Dec? Also suggestions for accommodation, transport and food will be helpful.

Good access from Tokyo. 1 hour and 40 minutes by Limited express car. All seats are reserved and very comfortable like Shinkansen. You will have a chance to see beautiful Mt. Fuji.
