My Number Notification Card and Application

I received my My Number notification card and want to send the enclosed application form off for a individual number card. I don't understand some things, though, so would appreciate some help.
1. Do I cut off the top part of the app form with number info and keep (actually, I did this already), just sending the rest?
2. Is there a 'Specimen' form somewhere on line that I could use to make sure I fill it in correctly. The only forms I can find are blank samples.
