usj to tokyo multiple transfers? how?

Please help!
I am leaving Universal City in Osaka to get to Tokyo.
I have looked up in Hyperdia timetable but it looks like there are multiple transfers 4-5.
I don't understand does that mean we must leave the train and get on another platform as well?
Please help!
Trying to find the best way to get back into Tokyo from Osaka!
I really appreciate any help!

Getting from USJ to Shin-Osaka Station takes 1-2 transfers. One at Osaka Station, and another one possibly at Nishikujo (where you just make a few steps across the platform), but some tranins go all the way to Osaka Station: https://www.usj.co.jp/e/access/From Shin-Osaka, you take the shinkansen to Tokyo where you may have to transfer again, depending on your final destination in Tokyo.
