From memory - they need to be purchased in person with evidence that the person is below the specific age. Others can confirm or clarify.

Buy in person because you have to verify age/eligibility otherwise anyone could buy a child pass. It is listed in the official how-to website, but it's been ages since I looked it up.

No.

For the reason mentioned by the previous posters,
you need to visit a ticket counter.

You show ID and verify the day of birth of that child,
so that the validity period of the card should be determined,
based on Japanese school years (from April to March).
