Hi,
I'm planning a trip to Hokkaido from Narita in December.
May I ask if the JR pass can be used for both transportations in Tokyo and Hokkaido?
Do I have to use a special pass just for Hokkaido?

It can be used on JR trains in both Tokyo and Hokkaido. It cannot be used on non-JR trains such as subways in Tokyo and Sapporo.

No.
If you want to travel from Narita to Hokkaido and are short of time, I strongly recommend to fly there instead of making the long journey by train. There are direct flights from Narita to Sapporo, which can be cheap if purchased in combination with an international flight or if used with another type of discount fare. Alternatively, there are lotsof flights from Haneda Airport to various airports across Hokkaido.
