There are many setups when it comes to withdrawals. My credit card works as following: No withdrawal fee, no currency exchange fee on top of Mastercards rate and the credit amount runs without interest until end of next month (when billed).
So it's really only the extra ATM fee of about 220 yen charged by the atm owner.
