Address and Reply?

Hello Everyone!

I was visiting Japan in November with my husband and we met this VERY nice and very generous Japanese lady on the train. She gave me her address and I would like to send her a postcard to thank her for being so generous (she shared her lunch and bought our train tickets!). She gave me her address but it's written in Japanese.
Is there anyone who could translate the address for me and provide a short message in Japanese I could write on the post-card? Something simple like "Hello from Canada, we met on the train on the way to Narita. We are so thankful for your generosity and help."

I have the paper she gave me and I can email it to you if you contact me.
