Quick Translation Question

Can someone kindly give me a natural Japanese rendering (in kanji/kana) for the following phrase?
"If found, please contact:"
Or "If this item is found, kindly notify:"
I want to put a little sticker with this statement followed by my local contact information (a Japanese resident's name and phone) on my JR pass and camera in case I stupidly drop one of them somewhere.
Thank you!!
