I don't think there is a want for outsiders to do this kind of work. Most of the worker Japans allow in are for roles that Japan's population cannot fill themselves. I'm not saying you cannot do the job in Japan. But you'd have to find and employer willing to employ you instead of a Japanese speaking Japanese resident.

I had a friend that did you personal training as a job in Japan. He was Australia born. but he was married to a Japanese ex-pat and could also speak the language.

Finding work probably wouldn't be a major issue presuming you speak Japanese pretty fluently. The bigger issue will be obtaining a visa, as there doesn't really seem to be a category that includes fitness instructor. You could bypass that step if you held some other resident status that allows unrestricted work (such as spouse of a Japanese national), however without that the chances to do this long term are probably very small if they exist at all.

There are actually quite a few foreign fitness instructors in Tokyo catering to expats who don't speak Japanese, but I think most of them are on spouse visas or have another residency status.

If you didn't mind teaching kids sports/fitness, I see occasional postings for English speaking PE teachers at private junior/senior high schools with immersion programs (you wouldn't necessarily need a teaching license) and there are some kid's gyms that are basically like sports and English after school care. It's not really personal training, but it is sports-related work that could get you an initial visa.

That still leaves the elephant in the room being the visa. The right kind of employer (say a gym that mainly caters to foreigners) might be able to spin it toward a specialist in humanities/international relations visa, but seems like a long shot. You might have more options if you are from a country that is part of the working holiday program. That would give you a year to establish yourself with a client base and then change to something like an investor or self sponsorship visa. Still kind of a long shot, and you should probably talk to someone more knowledgeable before embarking on this route.

You can absolutely work as a fitness instructor if you're fluent in Japanese. I was doing it myself as a part-time job while I was still a student.

Which visa will you get? I assume student or working holiday.
