I use a prescription shampoo for dandruff/itchy on my scalp. Is the shampoo something I have to declare and bring a doctor's note for or is it fine because it's just shampoo? Going there for vacation.

You would only have to declare it if it contained a controlled substance, which I highly doubt it does!
For peace of mind, just Google "controlled substances Japan", and you should be able to find a list.

You cannot bring it on a carry on luggage with the liquid allowances nowadays.
So it would need to go in your check in luggage. So make sure it is in a strong screw top bottle so the pressure doesn't queeze you shampoo through your suitcase.
