Work Visa, want to work part-time jobs

Here's my scenario. Because of family issues I want to be more flexible in that I may have to go back home sometimes to see my sick dad, and overall I have become burnt out. Currently I work 50 plus hours a week at my job. I've lived in Japan for almost 5 years and have 2 more years left on a 3 year Specialist in Humanities Visa. If I quit my job after finishing up my contract. I want to work part-time jobs. I would send in my "notification of the accepting organization (left the organization)" http://www.moj.go.jp/content/000099560.pdf to the immigration office. I would apply for National Pension, and National Health Insurance. Then because I would be applying and working at several part time jobs on and off I send immigration a "notification of the accepting organization" (of which I can't find which one that is on the Ministry of Justice website) http://www.moj.go.jp/content/001240106.pdf for each job I pick up OR don't send them one because I'll be free lancing? or what should I do? Thanks
