that is a master course (research course). you can't become a medical doctor after graduation. if you want to be a medical doctor, you have to enter undergraduate of medical schools.

I have some questions, what is your goal? Do you want to be working in Japan as doctor, or study and be doctor in your country or somewhere else?
I work in a pharmaceutical company in Nagoya and we do a lot with Tokyo university for research I know that English course is available.
If you want to work in Japan you need to know Japanese (Level 1, native) to work with patience. Even if you want to be caretaker for elderly people there is a whole process you need to pass before you even can support elderly people.

the license is only valid in that country, not worldwide. thus, in Japan, there is no reason to teach medicine in English.
if you learn it in English, go to UK, Australia, NZ, Canada, or USA.
