Concerning Change of residence

I'm trying to fill in the Change of residence status but I have some doubts about what to fill in.

Point 19 and 20.
Concerning place of employment, can I fill in the baito I'm doing now?

Concerning method of support, does this mean how I supported myself for the last months?

What type of visa you have? Yes you can change to other status too. From which category to which category you want to change?

Even if you have work you need to fill in point 19.
If not than NA (not applicable)

for 20 you need to full in who is supporting you if any.
If you have a spouse you fill in your spouse.
If you have another supporter in Japan you can fill in the other person.
If you have non than NA

I'm going from student to spouse.
But I'm not sure if what I have to fill in, concerns the past or the future.

If I get my spouse visa, I will change my baito, because I'll move.

Later in the document, they ask to write down the guarantor or contact in Japan.
Should I write my Spouse as contact here?

(As a Student, I have no guarantor in Japan yet.)
