2 Jobs and the gensen choshu hyo

Hi.
I currently work 3 days/week for company A and another 3 days/week for company B.
Should each of those companies issue me a "gensen choshu hyo" per year?
ps. both A and B is less than 20h/week each one. an average of 5h per day

Yes.
In case you do freelance, it would be a shiharaichosho 支払調書 rather than a gensen choshu hyo.
