If i have a JR east pass and opt for the Gran class, is it need to top up 18640 yen?

This seems to be a Shinkansen Gran Class ride.
Maybe for Tokyo -- Shin-Aomori?

Yes;
you pay 18640 yen,
and get not only a Shinkansen Super Express ticket
but also a Green ticket for Gran Class (A).
6800 yen paid as the Shinkansen surcharge
+ 11840 yen paid as the Gran Class (A) surcharge
= 18640 yen to pay additionally.
Your JR pass,
as long as it is valid for the section of your ride,
covers the basic fare.
