Kindly advise if there are any regional pass we can consider. My husband and I are spending 4 days in tokyo, 3 days in kawakuchiko, 3 days in nagano, 3 days in kyoto and 4 days in osaka.

For Tokyo: You can buy greater Tokyo pass 5 day=7200y Cover Tokyo yokohama narita haneda airport

4 days in tokyo, 3 days in kawakuchiko, 3 days in nagano, 3 days in kyoto and 4 days in osaka.
If you travel central Tokyo (mostly within Yamanote line), you don't need any rail pass. There are JR, Metro and "private" lines so your best bet will be an IC card. No discount but really convenient.
For Kawaguchiko, bus from Shinjuku is most convenient. Fast and cheap (2200 yen, 106 min).
If you mean Nagano for Nagano city (not Nagano prefecture), there are several ways to go from Kawaguchiko:
1. Bus to Kofu and then JR lines (Limited Express or Rapid) to Matsumoto and then to Nagano. 2. Back to Tokyo (or Omiya) and then take Shinkansen to Nagano. See google map.
from Nagano to Kyoto, Limited Express train to Nagoya and then Shinkansen to Kyoto, 11340 yen, 223 min.
All in all there seems to be no good pass which covers your itinerary.
