It's actually "Kanno-cho" rather than "Kamino-cho".
With some minor changes and the addition of the postal code, you would have...
000-00 Ishimori
Kanno-cho
Kakogawa-shi
Hyogo-ken 675-0005
Japan

I would expect the post code to have 7 numbers- it will get to its destination if you don't have the full post code but it might take slightly longer. It should look like this-000-0000

The postal code is there, 675-0005. I think the original poster just used 000-00 to mask the real house/street number, so don't worry.
