Is it better to get a Full Moon Pass as a married couple or to get a JR pass to visit Japan for 12 days? Do they include the Shinkansen? or we have to pay extra?

The Japan Rail Pass beats every single pass there is available for residents of Japan, including the Full Moon Pass.
Two 7-day Japan Rail Passes: 75,600 yen 5-day Full Moon Pass: 80,500 yen
Furthermore, the Full Moon Pass has some black out dates, while the Japan Rail Pass does not.
Go with the Japan Rail Pass, for sure.

As for coverage, the Full Moon Pass is virtually identical with the Green Japan Rail Pass.
