I think it's pretty difficult to tell the difference unless you are at an expensive place where you grate the wasabi yourself. At cheaper restaurants, it is unlikely to be 100% wasabi, but my view is: if you can't tell the difference, why worry about it?

It's actually quite easy to tell the difference. Real wasabi is not bright green, it has pulps and looks a little runny. Wasabi paste is more bright in color, smooth and pasty in texture. You can also see the chef grading real wasabi at a sushi bar if they are using the real thing.
