train fares/ ticket purchase

I am travelling to Japan in April. I will not have a rail pass as i will mainly be in Tokyo, it is my first trip. I am a little worried about getting the correct tickets when boarding trains subways etc are there ticket machines in English???? or do you purchase from a kiosk. Are there inspectors on every train and if so, if i have made an error in my ticket purchase, are there penaltys to pay or can you just pay extra to the inspector??
