Do I use Sama on gift tags?

I'm sending gifts to friends in Japan. On the gift tag, do I use just their names or Sama after the names, like when I address the envelope?

It depends on who the recipient is. If it is your friend, you can write "xxxさんへ" or "xxx(however you usually call him/her)へ".
