Dear You,
I am a mother of 5 months old baby (born in Toky), and have been here in Tokyo for about 1 year.
My husband and I still can not read Japanese Kanji-characters, therefore I am sending you this email for asking help about the vaccinations in Japan.
My baby has taken BCG vaccinations about 1 month ago (shoot at march 10,2010), and it's time to get the next DPT and Polio.
I received a mail of Polio and DPT vaccinations from ward-office, and found (maybe) questionnaires that should be filled before taking the vaccinations for my baby. I have asked people in ward-office, to send me the English version of the questionnaire, but unfortunately, they don't have any.
So please, if somebody knows, where can I get the English version of questionnaire for vaccinations, please inform me.
I have also contacted several pediatrics for the vaccinations, but no good response from them... :(
Please help me....
