Pension Tax Proxy form

Hi was wondering if anyone one knows how to fill out the 納税管理人の届出書 form. The one where you nominate a person in Japan to act on your behalf to claim the 20% tax on your pension. The form is all in Japanese. I found one website with some of the sections translated into English for a guide but it only translated some of the parts.
